Transition Support Service

The Rainbow Place Transition Support Service is designed to assist young adults with life-limiting conditions as they move from paediatric to adult healthcare services. This may be due to changes in eligibility for paediatric support, the need for adult services to continue medical care, or relocation.
Led by a Clinical Nurse Specialist (CNS), the service provides structured, individualised support to ensure a seamless and well-coordinated transition for both the young person and their family.
The Transition Service includes:
• Comprehensive transition planning that addresses ongoing medical, psychosocial, and practical needs
• Navigation support to help families understand and access appropriate adult healthcare services and community resources
• Development of personalised transition plans, including Advanced Care Plans and Goals of Care discussions
• Coordination between paediatric and adult service providers, ensuring continuity of care and shared understanding
• Emotional and psychosocial support for young people and their families as they adjust to new systems and responsibilities
This service ensures that every young person approaching transition receives the care, guidance, and advocacy needed to move forward with confidence and clarity.
