Community Nursing Support

Rainbow Place’s dedicated Community Nurses and Clinical Nurse Specialist work in partnership with families, delivering personalised care and support within the comfort of their own homes.

The team maintains regular contact through a combination of home visits and phone calls, travelling across the Waikato to ensure families feel supported and connected. A holistic approach to care supports not just the child, but the entire family—recognising the unique emotional, practical, and physical needs of everyone involved and acknowledging the isolation these families can face.

Time is taken to truly understand each family's preferences, values, and circumstances, placing them at the heart of every decision. The goal is to empower families with choice and ensure they feel heard, respected, and cared for.

Community nursing support includes:

• Comprehensive needs assessment and tailored support for the whole family
• Practical guidance and care strategies to manage daily challenges
• Advocacy for the family and child at appointments, in school meetings, with funding and care supports
• Education for schools and other supports for complex care and management
• Ongoing emotional support, offering space to express fears, share hopes, and make informed decisions about their child’s care
