Tō tātou reo advance care planning is about taking a moment to think about what matters most to you — your values, your wishes, and the care you would want if you became unwell and couldn’t speak for yourself. It is a national programme led by Health New Zealand and Hospice Waikato holds the contract to deliver this service across the Waikato region.

Advance care planning is not about making decisions all at once or planning for the worst. It’s about having gentle conversations, at your own pace, so the people who care for you — your whānau and your health team — understand what’s important to you. These conversations can feel big, but they don’t need to be overwhelming. You can start wherever you are and take things one step at a time.
