End of Life Care

Rainbow Place provides compassionate, comprehensive end-of-life paediatric palliative care in collaboration with each child’s existing healthcare team. The focus is on co-ordinating and managing complex symptoms and delivering holistic support to the child and their family throughout the final days and weeks of life.

Working closely with the child, their family, and healthcare professionals, care is aligned with the families and child’s values, preferences, and end-of-life wishes.

End-of-life care includes:

• Advance care planning – Supporting children, young people, and their families in expressing and documenting their wishes and priorities for end-of-life care
• Expert multidisciplinary team coordination – Access to a skilled and experienced team of nurses, doctors, therapists, and support staff
• Individualised care plans – Tailored care to manage pain, alleviate symptoms, and maintain comfort and dignity
• Nurse-led clinical support – Compassionate, responsive care addressing physical needs and symptoms
• Practical guidance – Assistance with funeral planning, death registration, and coordination with relevant agencies
• Bereavement support – Emotional, cultural and psychological care before and after loss, including one-to-one and family support
• Memory-making activities – Creating lasting keepsakes and special experiences, including photography and other meaningful moments

Care is centred around the individual needs of each child and family, offering dignity, comfort, and support at every stage of the journey.
